Patient Care Advocates is a Medicare certified Home Health organization. We provide care for a wide range of medical conditions. Some examples are diabetes, diseases, infections, care needed as a result of surgery, injuries, and wound care. We also help you with your medications, regularly re-evaluate your medical needs, and provide you with preventative and rehabilitative nursing and therapy. Our team of experts is comprised of Physical Therapists, Registered Nurses, Occupational Therapists, Social Workers, Certified Nursing Assistants and Speech Language Pathologists.

Our unique Hospital Aid Services, Provides an RN to meet you in the ER within 75 minutes of being informed that you are going to the hospital to monitor your treatment and review your medications

When You Arrive At The Hospital

• A Patient Care Advocates Registered Nurse will meet you after your arrival at the hospital
• She will check to see that you’re getting attention…taking the stress out of your stay
• Once you are settled in, nurses will continue to monitor you and your care a Patient Care Advocates Caregiver will be there to help monitor your progress and provide you with old fashioned comfort care.
