Soulistic Hospice is a non profit 501(c) (3) service providing quality care for loved ones who qualify and are declining in health. Qualifications include having life limiting illness such as Heart, Pulmonary, Kidney or Liver Disease, Cancer, Diabetes, Dementia, Parkinson’s, or stroke, including frequent falls, uncontrolled or increased pain, frequent infections, frequent visits to hospital or doctor’s office, shortness of breath, losing weight and/or interest in food, increased weakness and sleeping, Covered 100% by Medicare, AHCCCS and most private insurances. We strive to provide hospice care regardless of ability to pay. We offer incontinent supplies, medical equipment, in home Nurse visits, CNA assistance for bathing and light housekeeping, as well as physical, emotional, and spiritual needs.
