United Community Health Centers

United Community Health Center (UCHC), a Federally Qualified Health Center (FQHC), is a medical home that provides primary and preventive health care to the population of southern Arizona and is open to all regardless of their ability to pay. We have Enrollment Specialists who assist with applications for Medical Assistance, Nutrition Assistance and Cash Assistance. They are dedicated to patients in need of assistance to personally guide them through the qualification of County, State and Federal programs. Such as:
AHCCCS (Arizona Health Care Cost Containment System) AKA Medicaid
SNAP (Supplemental Nutrition Assistance Program) AKA Food Stamps
TANF (Temporary Assistance for Needy Families) AKA Cash Assistance
Kids Care (Children’s Health Insurance Program) AKA CHIP
Freedom to Work- Medical for Disabled adults
Medicare Cost Sharing- Assistance with Medicare premium, deductibles, co-pays
Federal Market Place Enrollment through the ACA (Affordable Care Act)
Currently, there are eleven community health centers under the umbrella of UCHC located in the communities of Green Valley, Sahuarita, Vail, Arivaca, Amado, and Three Points.
